- Get the MOAPS form from the District Social Welfare Office or download it from the department website.
- Fill in your details, have it recommended by a local leader or Panchayat secretary, and submit it to the District Social Welfare Officer during the notified period.
- Attach Aadhaar, bank details, an income certificate from the SDO or SDC (or NFSA card details), two passport photos and an age proof.
