Health insurance claim rejected? Your grievance letter, filled in for you
If your health insurance claim was rejected or paid short, first write to the insurer's Grievance Redressal Officer quoting the policy and claim numbers, the reason given and why you disagree. If there is no reply in the time IRDAI sets or you are not satisfied, escalate on Bima Bharosa and then go to the Insurance Ombudsman, which is free.

This is a general format to save you time, not legal advice. Read every clause, change what does not fit your situation, and for a dispute or a large amount, consult a lawyer.
What is a health insurance claim grievance?
A formal grievance to the insurer. It lists your policy and claim details, the reason the insurer gave, why that reason does not apply (with the IRDAI rule where one helps), and asks for a written reply with the policy clause relied on. It also says you will go to Bima Bharosa and the Insurance Ombudsman if needed.
When do you need a health insurance claim grievance?
- Your claim was rejected.
- The insurer paid much less than the bill without clear reasons.
- The claim has been pending for a long time.
Documents needed for a health insurance claim grievance
- The insurer's rejection or settlement letter.
- Discharge summary, final bill and payment receipts.
- A note from the treating doctor, if the reason is medical.
- Policy schedule and wording.
How to make a health insurance claim grievance: stamp paper, signing, submitting
- Fill in the insurer, policy and claim numbers, and the reason the insurer gave.
- Write in two or three lines why you disagree, in plain words.
- Email it to the grievance address on the insurer's website or submit it at a branch; keep the complaint number.
- If not resolved, escalate on bimabharosa.irdai.gov.in, then complain to the Insurance Ombudsman at cioins.co.in within one year of the insurer's reply.
Common mistakes
- Going to the Ombudsman before writing to the insurer: the insurer must get a chance first.
- Not keeping the complaint number and date.
- Missing the one-year limit for the Ombudsman.
Questions people ask
Does the Ombudsman charge a fee?
No. The Insurance Ombudsman is free, and you do not need a lawyer. It hears complaints up to ₹50 lakh.
How long does the insurer have to reply?
IRDAI requires insurers to resolve grievances within the time set in its policyholder protection rules; the insurer's grievance page shows it. If there is no reply in 30 days, you can go to the Ombudsman.
