For pensioners who must prove every year that they are alive to keep getting pension. Central government pensioners usually submit in November; those aged 80 or more can submit from 1 October.

Aadhaar number
Your own Aadhaar. Your face or fingerprint is matched with it, so it must be the pensioner’s, not a child’s.
Name (as in Aadhaar)
Exactly as on Aadhaar.
Mobile number
The certificate ID comes here by SMS. Keep it.
PPO number
Pension Payment Order number from your pension order or the pension slip. Copy every letter and slash.

Common mistake: A mistyped PPO: the certificate reaches nobody and pension stops.
Type of pension
Service, family, or other pension as on your order.
Sanctioning authority
The department or organisation that approved your pension (for example a central ministry, state government, EPFO, railways, defence).
Disbursing agency
The bank or post office that pays your pension.
Pension account number
The account where pension comes.
Proof: Bank passbook (first page)
Re-employed or re-married
Answer Yes or No truthfully. For family pension, re-marriage can change eligibility.
Why forms get rejected
- Fingerprint not matching for older people: use face authentication on the phone instead, or a doorstep service.
- Submitting late: pension can stop from the next month until the certificate arrives.
- Calls asking for OTP "to update life certificate": the bank never asks for OTP on call.
