Ration card application (NFSA) box by box
For a household applying for a new ration card, adding or removing members, or moving from another state. The form is the state’s own, but the boxes are much the same everywhere.

Head of the family
Under the National Food Security Act, the eldest woman of 18 or more is the head of the household for the card. Only if there is none, a man is head.
Common mistake: Writing the man of the house as head when an adult woman lives in the family.
Family members (name, relation, age, Aadhaar)
Everyone who lives and eats together. Each member’s Aadhaar is linked for rations. A person can be on only one ration card.
Common mistake: A member still listed on a parent’s or old card: remove the name there first.
Aadhaar number of the head
12 digits. The name must match the Aadhaar for biometric ration at the shop.
Address
Where the family lives now. Your fair price shop is assigned from this.
Proof: Electricity bill, rent agreement, voter ID, Aadhaar
PIN code
6 digits.
Annual family income and occupation
Total income of everyone in a year, in rupees. States use it to decide AAY or priority household. Write the truth: wrong income can cancel the card and lead to recovery.
Proof: Income certificate from the tehsil, if the state asks
LPG connectionif it applies
Consumer number and agency, if you have one. Some states reduce kerosene for gas households.
Bank account and IFSCif it applies
Some states pay cash transfers in place of grain. Copy the IFSC from your passbook.

If you had a card in another district or state, surrender it there first. With One Nation One Ration Card you may not need a new card at all: you can take rations at any shop with your old card.
Why forms get rejected
- A member on two cards (for example a married daughter still on her parents’ card).
- Aadhaar of members not seeded, so the shop machine refuses rations.
- Income certificate missing where the state requires it.
