9 boxes · tap the speaker to hear any box
  1. Aadhaar number (only for update)if it applies

Leave blank for a new enrolment. For an update, write your 12-digit number.

  2. Full name

Exactly as on your proof of identity, no Shri or Smt. A name can be changed only twice in a lifetime, so get it right.

Proof: Proof of identity: passport, PAN, voter ID, ration card with photo, driving licence, school certificate with photo

Common mistake: Name on the form and the proof do not match letter for letter.

  3. Gender

Male, Female or Transgender. Can be changed only once.

  4. Date of birth (declared or verified)

Tick "verified" only if you bring a date of birth proof; otherwise it is "declared". Date of birth can be changed only once, and only within limits.

    Example: 15/01/1985

Proof: Birth certificate, Class 10 marksheet, passport, pension order

  5. Address (C/O, house, street, village, post office, district, state)

Write it the way your address proof shows it. C/O (care of) is optional for adults.

Proof: Proof of address: bank passbook, electricity, water or gas bill, ration card, rent agreement, voter ID, or a certificate from a gazetted officer, MP, MLA or village head on the UIDAI format

  6. PIN code

    6 digits.

  7. Mobile number

A number you keep. Every Aadhaar OTP (bank, PAN, DigiLocker) goes here. Mobile changes need a centre visit, not an online form.

  8. Emailif it applies

    Optional.

  9. Head of family or relationship details (children and those without own proof)if it applies

For a child under 5, a parent’s Aadhaar is linked. For an adult without own proof, the head of family’s Aadhaar and a document showing the relation (ration card, marriage certificate) are used.

Why forms get rejected

  • Wrong form: the UIDAI form has different versions for residents above 18, children 5 to 18, children under 5 and NRIs. Ask for the right one at the centre.
  • Proof document in a different spelling or older address.
  • Paying a middleman: forms are free and fees are fixed by UIDAI; always take the printed acknowledgement slip.
  • Children not updating biometrics at 5 and 15: the Aadhaar can stop working.

How it works

  • We explain the boxes and link the official portal. We never submit anything or collect your details.
  • Your practice answers stay in this tab. They are not sent anywhere, and they are gone when you close the tab unless you save a copy yourself.
  • Examples are made up. Copy numbers from your own documents, not from memory.
