12 boxes · tap the speaker to hear any box
  1. 1. Name and surname

Write your name exactly as on your Aadhaar or birth proof, in English and in your state language. If you fill only one, the system converts it and spellings often go wrong.

    Example: Sunita / Devi

Proof: Birth certificate, Class 10 or 12 marksheet with date of birth, Aadhaar, PAN, passport or driving licence

Common mistake: Nicknames or a different spelling from Aadhaar.

  2. 2. Name of relative (father, mother, husband, wife, guardian)

Tick which relation it is and strike out the others. A married woman may give her husband’s name.

    Example: Father: Ramesh Kumar

  3. 3. Mobile number and emailif it applies

Your own or a relative’s mobile. Status updates and the e-EPIC download OTP come here.

    Example: 98765 43210

  4. 5. Aadhaar numberif it applies

Give it if you have it: it is used only to check your entries. If you have no Aadhaar, tick the box in item 5(b) instead. The form is not rejected for this.

  5. 6. Gender

Tick Male, Female or Third Gender. A third gender person may choose any of the three.

  6. 7. Date of birth

Write it as DD/MM/YYYY and attach a self-attested copy of one listed proof. With any other document you must appear in person before the ERO.

    Example: 05/08/2007

Proof: Birth certificate, Class 10 or 12 marksheet with date of birth, Aadhaar, PAN, passport or driving licence

Common mistake: Date on the form differs from the date on the proof.

  7. 8. Present ordinary residence

Where you actually live now: house number, street, village or town, post office, tehsil, district. Students may use the hostel address or their parents’ address. A booth officer may visit to check.

Example: H.No. 12, Ward 4, Rampur, PO Rampur, Tehsil Sadar

Proof: Water, electricity or gas bill (one year or newer), bank or post office passbook, Aadhaar, passport, registered rent deed or sale deed, in your own name or a parent’s or spouse’s

Common mistake: Giving a native village address when you live and work elsewhere: the BLO finds you absent.

  8. PIN code

The 6-digit PIN of your post office. Check it on our IFSC and PIN finder if unsure.

    Example: 226001

  9. 9. Disability (if any)if it applies

Tick the type and attach the disability certificate if you have one. This gets you help at the booth (ramp, wheelchair, home voting where allowed).

  10. 10. Family member already on the list at this addressif it applies

Name, relation and EPIC (voter card) number of a family member already enrolled here. It helps the ERO find your house part of the list.

    Example: Ramesh Kumar, father, ABC1234567

  11. Declaration: place, date, since when living here

Fill every line: the date from which you live at this address and that you are not enrolled anywhere else. A false statement is punishable under section 31 of the Representation of the People Act, 1950.

  12. Photograph

Recent colour photo, 4.5 cm x 3.5 cm, white background, eyes open, both edges of the face visible. Do not sign on it.

Why forms get rejected

  • Already enrolled at the old address: use Form 8 (shifting) instead of a fresh Form 6, or you may be on two lists.
  • Name spelling in Hindi and English differ, or only one language filled.
  • Address proof not in your, your parent’s or your spouse’s name.
  • Not at home when the BLO visits, and no phone number given.
  • Unclear or signed photo.

How it works

  • We explain the boxes and link the official portal. We never submit anything or collect your details.
  • Your practice answers stay in this tab. They are not sent anywhere, and they are gone when you close the tab unless you save a copy yourself.
  • Examples are made up. Copy numbers from your own documents, not from memory.
