Form 8: shift, correct, replace voter card, mark PwD box by box
For people already on the voter list who moved house, need a correction (name, age, photo, relative), want a replacement card, or want to be marked as a person with disability.

EPIC (voter card) number
Copy it from your card or find it on the ECI search site. It tells the ERO which entry to change.
Example:

As it is on the voter list now (even if you are correcting it).
What you want (tick one)
Pick one: shifting of residence, correction of entries, replacement card without correction, or marking as person with disability. Fill only that part.
Common mistake: Ticking more than one option, or using Form 8 when you are not on any list (use Form 6).
New address (for shifting)if it applies
Full new address with PIN, and a proof of it. Shifting inside the same constituency and to another one both use Form 8 now.
Proof: Water, electricity or gas bill (one year or newer), bank or post office passbook, Aadhaar, passport, registered rent deed or sale deed, in your own name or a parent’s or spouse’s

6-digit PIN of the new address.
Entry to correct and the right detailsif it applies
Tick the entry (name, gender, date of birth, age, relative, photo, address details) and write only the correct value, with a document that shows it.
Common mistake: Asking to correct a date of birth without a document that shows the new date.
Mobile numberif it applies
For status updates and the e-EPIC OTP.
Why forms get rejected
- Using Form 6 when you are already enrolled: duplicate entries get deleted and can delay both.
- No proof for the corrected value or the new address.
- Wrong EPIC number, so the request goes to someone else’s entry.
