Given name
Your first and middle names, without surname. If you have only one name, write it here and leave surname blank.
Example:

Surnameif it applies
Family name only.

If you were ever known by another name (before marriage, a spelling in old certificates), say Yes and give it. Hiding it causes police verification trouble.
Date of birth
DD/MM/YYYY, the same as your date of birth proof.
Proof: Birth certificate (must for those born on or after 1 October 2023), Class 10 marksheet, PAN, Aadhaar, driving licence, voter ID
Place of birth (village or town, district, state)
Write the place as your birth proof shows it. If born outside India, the country.
Father’s, mother’s, guardian’s name
Given name and surname as on their documents. For a single parent, only that parent’s name is enough.
Spouse’s nameif it applies
Only if married. A marriage certificate is not needed to add a spouse name.
Educational qualification and ECR
Class 10 pass or above makes you non-ECR (no emigration check). Bring the marksheet to prove it.
Present address
Where police verification will happen. You must have lived there at least a year, or also give the earlier address.
Proof: Aadhaar, bank passbook with photo, utility bill, rent agreement, spouse’s passport
PIN code
6 digits.
Mobile number
SMS updates and police contact.
Emergency contact
Name, address and phone of someone who can be reached if something happens to you abroad.
Other details (court cases)
Answer truthfully. A pending case needs court permission, but hiding it can lead to refusal or a fine.
Why forms get rejected
- Name or date of birth differs between documents: fix the documents first or bring the annexure the booklet asks for.
- Applying through unofficial sites that look like Passport Seva and charge extra.
- Address where you do not actually live: police verification comes back adverse.
- Not bringing originals of every document to the appointment.
